1. Recurrent thoughts of and preoccupation with death.
2. Recurrent or ongoing suicidal ideation without any plans.
3. Ongoing suicidal ideation with a specific plan.
4. Recent suicide attempt.
5. Family history
6. History of suicide attempts that required professional intervention.
7. Social with drawl, lethargy and apathy.
8. Engages in self-destructive behavior.
9. Self esteem, attitude coupled with recent life events that support this.

Long-Term Goals:
1. Alleviate the suicidal thoughts/ideation and return to the highest level of previous life functioning.
2. Stabilize the suicidal crisis
3. Placement in an appropriate level of care to safely address the suicidal crisis.
4. Self development, introspection and enhance self-esteem
5. Resolve emotional conflict(s) and adjust to life events.

Short-Term Objectives & Therapeutic Interventions:
1. State the strength of suicidal feelings, frequency of thoughts and detail of plans.
* Assess the client's suicidal ideation taking into account extent of ideation, presence of primary and backup plan, past attempts, family history.
* Assess and monitor client's suicidal potential on an ongoing basis.
* Notify the resident's family/significant others.
* Form a 24-hr suicide watch until the crisis subsides
* May administer Minnesota Multiphasic Personality Inventory(MMPI),
Beck Depression Inventory(BDI) or Modified Scale for Suicide Ideation(MSSI).
2. Verbalize a promise to contact the therapist or some other emergency helpline if serious harm toward self harm arises.
* Elicit a promise from client that he/she will initiate contact in such situation.
* Provide the client with an emergency helpline telephone no. available 24/7.
* Make a contract with the client identifying what he/she will and won't do in such situation.
* Offer to be available to the client through tel. contact if a life threatening
