ANS 1:
Following are the steps I will take after receiving Functional Ability Assessment form:
- I will conduct a functional needs analysis to support the documentation for accommodation.

- Moving further I will generate and access creative accommodation options for employee.
- After that I will access medical impact, workplace impact and contractual impact so right decision can be made.

ANS 2:
Following are the activities which Jonathan may be unable to carry out at work based on his job description and outside at work:
- He cannot operate the vehicle manually.
- He cannot load and unload the truck.
- He cannot climb into truck cab.
- He cannot grip, lift, push, pull and carry wheeled, cardboard boxes over, up and down inclines, stairs, changes in walking surfaces and elevations.
- He cannot lift 70 lbs.
- He cannot walk, stand, and sit for extended periods of time.
ANS 3:
Jonathan is entitled for workers’ compensation because:
- He has worker-employer relationship with the company and the employer is covered by the WSIB.
- He has sustained work related injury so he is entitled to be compensated for loss of earnings and loss of functional ability.
- He has provided the Functional ability assessment report.
ANS 4:
Following are the sources and people I would consult in order to develop a Return to Work Plan:
- Employer
- Injured worker
- Health care provider
- Insurance provider
- Government (if required)
- Disability management contractors
ANS 5:
- As per Jonathan duties in delivery driver, he has to prepare accurate paperwork and records and he has to communicate in a friendly professional manner with clients and co-workers, so according to my research and considering the worker’s restrictions and limitations he can perform the duty of dispatcher.
The reason for being is that the job of dispatcher does not require physical effort as lifting, standing etc. It just require knowledge of the routes to scheduling the deliveries which he has, basic knowledge to prepare paper work regarding shipping, receiving, dispatching daily orders. Communication skills to communicate in friendly professional manner with clients and coworkers.
- The type of accommodation which is suitable for the worker is Light duty work and gradual work exposure. The reason being as he cannot work for his regular job i.e. delivery driver, for gradual work exposure is because as Jonathan cannot sit, stand and walk for the extended period of time. It will provide him light duty work where hours and expected duties slowly increase as his injuries will take a month to recover and his ankle will take a longer time to recover. Duties he will perform:
- He will do the dispatching and receiving orders for deliveries.
- Enter the data in computer, maintain call logs and other information.
- Prepare the paper work regarding shipping, receiving and deliveries.
- Communicate with client and customers in professional manner.
- Monitor the route and scheduling the deliveries.
d) I will monitor on his health by talking to his physician on time to time basis and also review his progress report on the injury and also talk to him regarding his progress.
