Nursing Diagnosis

Injury, risk for/actual (trauma, suffocation, poisoning)

Interventions

1. Acquire information about nature of emergency, accident or disaster.

2. Prepare are and equipment, check and restock supplies.

3. Assist in prioritizing (triaging) patients for treatment. Monitor for /treat life threatening injuries

4. Determine primary needs/ specific complaints of the patient. Check for medical tag alert.

5. Obtain additional medical information including preexisting conditions, allergies, and current medication. Perform more in-depth assessment as time allows/condition warrants.

6. Determine patient's developmental level, decision-making ability, level of cognition, and competence.

7. Evaluate individual's response to the event, mood, coping abilities, personal vulnerability.

8. Ascertain knowledge of needs/injury prevention and motivation to prevent further injury.

9. Identify/ manage life threatening situations

10. Discuss importance of self monitoring of conditions.

Infection, risk for Interventions

Interventions

1. Note risk factors for occurrence of infection (e.g. environmental exposure, comprised host, traumatic injury/ loss of skin integrity). Determine proximity to incident. Be aware of incubation period to various diseases.

2. Observe signs and symptoms of infective agent and sepsis; fever chills, diaphoresis, altered level of conscious, positive blood cultures. Investigate presence of rah.

3. Practice and demonstrate proper hand washing skills.

4. Provide for infection precautions/ isolation as indicated.

5. Instruct patient in techniques to prevent spread of infection, protect the integrity of skin, and care of wounds/lesions.

6. Obtain appropriate specimens for observation and culture/sensitivity testing.

7. Administer monitor medication regimen.

Post-Trauma Syndrome, risk for

Interventions

1. Determine involvement in event.

2. Evaluate life factors/stressors currently or recently occurring such as displacement from home due to catastrophic event. Identify how patient's past experiences may affect current situation.

3. Listen foe comments of taking responsibility (e.g. "I should have been more careful/ gone back to get her.)

4. Identify patient's current coping mechanisms.

5. Determine availability/ usefulness of patients supports systems.

6. Provide information about signs and systems of post trauma response, especially in individual is involved in a high-risk occupation.

7. Identify and discuss patient's strengths as well as vulnerabilities.
