Child Function Report

1. Please describe the child's activities during a typical day, from the time he or she gets up until bedtime, including any problems with sleeping.

I would usually wake Sarah and her brother at 7:00am during the school week. She would cry and say; "I hate school, school is bad". Everyday I would reply by asking:" Why do you hate school? She replies: Because I'm scared. Then I would ask: Why are you scared? She reply's: "because school is bad. Every morning we would start the day with those sentences.

She would then go potty and then I would remind her to wash her hands and brush her teeth. I usually put out breakfast for Sarah and her baby brother Christian at about 7:45am. Then at about 8:15am, we would walk to the school bus stop.

As soon as we are at the school bus stop, she would ask: "Do you wanna use my can opener, for this jar of mayonnaise? And phone books? I usually just play along so that the other kids at the bus stop wouldn't laugh at her.

When we see the bus coming down the street, I would tell her to get on line. She usually asks, "Where is the line mommy? She will only get on line when it is already formed. Sarah doesn't ever start the school bus line.

3:30pmÐ'...

I pick her up at the bus stop, she is usually very glad to see me and her brother. We would start walking home, then on the way we stop by the leasing office for the cookies. Then we arrive home and fix Sarah and her brother a snack.

4:30

Sarah and I would do homework. I usually have to explain it to her in a way that she could understand, ex:

4+5=9

I would have to tell her to draw four little circles and five more circles. She would then say, 9? Then I would say, that's your answer honey!

5:00pmÐ'...

Sarah and Christian get to watch TV while I make dinner. Around this time, Sarah's dad arrives home from work. Sometimes she greats him by saying," Hi Dad! What's your favorite color? He usually just plays along and tells her blue.

6:00pmÐ'...

Dinner is ready, I always make chicken or fish but, for the kids its either chicken nuggets or fish sticks with spaghetti. Sarah loves to eat salads, almost as much as French Fries.

7:00pmÐ'...

The whole family sits down to watch TV

8:00pm

The kids are given a shower by either my husband or me.

9:00pm

Both Sarah and Christian are read a story by there dad, then they are off to bed.

Even though the kids each have there own separate beds, they prefer to sleep in the same bed together. Sarah will cry if we put them in separate beds.

2. What kind of help, if any, do you provide the child with getting up, getting dressed and ready for the day?

I'd wake her up, tell her to brush her teeth for sixty seconds, and then pick out her clothes. If I allow her to choose her own outfit, she would choose to wear shorts and a t-shirt in the middle of winter. I usually style her hair by putting it into a ponytail; this is the only way she wants to wear it, if I try a different hair style she gets upset and cries.

3. In what school activities does the child participate? If none, please tell why.

She is in a regular 1st grade class but, she gets Special Education English and Math, also she receives Occupational therapy and Speech therapy 2 per week.

4. How independent is the child in completing assignments, such as homework? What kind of help do you provide, if any? I usually need to explain her homework in great detail to her.

5. Please describe the child's activities outside of the school environment with friends. Sarah doesn't have any friends. We usually just stay home or go to the store.

6. Has the child been given the responsibility of performing chores around the house? If yes, how does he/she handle the responsibility? I have tried to get Sarah to make her bed but, she is unable to complete the task. She would just throw the covers on the bed and says "All done"!

7. Describe the child's ability to care for him/her (e.g. eating, dressing, bathing, safety, etc.) Sarah is able to feed herself. If I choose her clothes, she can dress herself. She is unable to choose weather appropriate clothing. In winter she would put on a pair of shorts and a t-shirt. Sarah is not able to bathe herself, and if unsupervised, she does not wash her hands properly after using the bathroom.

8. Describe the child's ability to express his/her needs and to carry on a conversation. Sarah has trouble carrying a meaningful conversation. She often repeats lines from her TV shows, fantasy, or storybooks. I'll say: Sarah, what do you want to do today? She would reply," Lets go to the crusty
